An engolpion, medallion, with the icon of Jesus or a saint is worn in autocephalus (autonomous) Orthodox churches by all primates (chief bishops) and some other bishops. This one shows Saint Nicholas. It is worn in addition to a engolpion depicting the Theotokus, Mother of Jesus, called a Panagia.
