The wooden church was built in the early 16th century and rebuilt in baroque style in 1658. It was the parish church in Polanka Wielka until 1991 when the new church, Nativity of the Blessed Virgin Mary, became the parish church. The church is on the Wooden Architecture Trail of the Lesser Poland Voivodeship. The church is built of logs with paneling. The altar is from the 1700s and there are interior paintings from the 16th and 17th centuries.
