The filial chapel was built in 1793 in Wiezanka and moved to Klejniki in 1834 where it served as a parish church. It moved again around 1900 to Kozliki. The chapel belongs to the Klejniki parish. The Narew River, threatening the chapel, led to strengthening the river bank and the church foundations from 2013-2016. The whole chapel was raised 70 cm and the fence repaired. It was consecrated in 2016. It is a listed monument.
